Adapt Sports Physiotherapy (“we”, “us”, “our”) is committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). As a private-sector health service provider, we are bound by the APPs regardless of annual turnover — the small business exemption does not apply to health service providers.
This policy covers information collected through this website only. It does not describe how we handle your clinical or health records, which are collected and managed separately as part of your care. If you have questions about your clinical records, please contact us directly.
What we collect through this website
If you use our website enquiry form, we collect the information you choose to provide: your name, email address, phone number, and a brief message about what you’d like help with. We do not ask for, and you should not include, details about your medical condition or symptoms in this form — it is a request for contact, not a clinical intake form.
We do not use cookies for analytics, advertising or tracking on this website.
How we use and store it
Your enquiry is sent to us by email, using a secure email service, so we can respond to you. We do not store your submission in our website’s database — once your message has been emailed to us, the record is automatically and immediately deleted from the website.
Once received by email, your enquiry is kept for as long as reasonably necessary to respond to your enquiry.
Booking appointments
Booking an appointment takes place through Cliniko, our practice management software, which operates independently of this website. This policy does not cover information you provide directly to Cliniko when booking — please refer to Cliniko’s own privacy policy for that.
Other links from this site
Our website links to our Google Business Profile and our social media profiles (Facebook, Instagram, YouTube). Visiting those sites is governed by their own privacy policies, not this one.
Access, correction and deletion
You can ask us what personal information we hold about you, ask us to correct it, or ask us to delete it, by contacting us at admin@adaptsports.physio.
Complaints
If you have a concern about how we’ve handled your personal information, please contact us first at admin@adaptsports.physio. If you’re not satisfied with our response, you can contact the Office of the Australian Information Commissioner (OAIC).
